Entries will be accepted through September 30, 2009. Individuals can enter by visiting a participating Chevy dealership and completing an offcial entry form; or by completing a Healthy Air Living Pledge Card Entry Form available at www.healthyairliving.com and in Valley Air District offces. No purchase necessary. One entry per person. Must be 18 years or older, a licensed CA driver and a resident of the San Joaquin Valley Air Pollution Control District. No photocopies, facsimiles, labeled entries, or stamped entries will be accepted. Only completed entry forms with entrant’s name, complete mailing address, including street, city, and zip code and phone number, will be accepted. Illegible entries will not be considered. Prize is non-transferable. Deadline to enter is 5:00 pm on September 30, 2009. Employees and immediate family members of Chevrolet, its subsidiaries, Chevy Dealerships, Chevy’s advertising agency, Jeffrey Scott Agency and the San Joaquin Valley Air Pollution Control District are ineligible. Winner will be drawn at random and formally announced on November 5, 2009, at a public meeting of the San Joaquin Valley Air Pollution Control District’s Governing Board. Winner will be required to sign release forms before receiving prize. Proof of age and automobile insurance are required. Tax, license, registration and any other related fees will be paid by the winner. Odds of winning depend on the number of entries received. To request name of winner, send a self-addressed, stamped envelope to the San Joaquin Valley Air Pollution Control District, 1990 E. Gettysburg Ave., Fresno, CA 93726-0244 after October 15, 2009. All entries are subject to eligibility verification by the San Joaquin Valley Air Pollution Control District and to all applicable local, state and federal laws.
